Clinical Utility and/or Limitations CYP2D6 is the major CYP isoform involved in tamoxifen metabolism to the antiestrogen endoxifen. CYP2D6 genetic variation and inhibitors of the enzyme can markedly reduce endoxifen plasma concentrations in tamoxifen-treated patients. In fact, CYP2D6 has been shown to be an independent predictive factor in women with breast cancer treated with tamoxifen. Tamoxifen-treated patients who are poor or intermediate metabolizers (associated with impaired anti-estrogen metabolite production) had an increased incidence of recurrence and shorter relapsefree periods.
