CYP2D6 is the major CYP isoform involved in tamoxifen metabolism to the antiestrogen endoxifen. CYP2D6 genetic variation and inhibitors of
the enzyme can markedly reduce endoxifen plasma concentrations in tamoxifen-treated patients. In fact, CYP2D6 has been shown to be an
independent predictive factor in women with breast cancer treated with tamoxifen. Tamoxifen-treated patients who are poor or intermediate
metabolizers (associated with impaired anti-estrogen metabolite production) had an increased incidence of recurrence and shorter relapsefree
periods. |
